Professional Program Application
To apply for admission into the Professional Program or to request permission to take Professional Program courses, complete the form below:
To be authorized to add Professional Program courses you are requesting by Priority Registration week, this form must be submitted by November 1 for fall semester and April 1 for spring semester.
Approval of your request to take Professional Program courses is dependent on you passing pre-requisite courses and meeting admission/permission requirements (contact your advisor for questions about requirements). If after this semester you have not passed your courses and/or met the requirements, you will be dropped from the applicable Professional Program courses.
This form is checked infrequently after posted deadlines and during the summer. If you fill out the form during those time periods, please contact your advisor via email immediately afterwards.
